Enroll at the San Jose Math Circle

We prefer online submission but you may, if you wish, fill out a paper form, which you should bring at the first meeting.

Before you fill in the form (below), please read the following information carefully:
  • Make sure you type your name and email address correctly, as we will use both to identify you, so double check, please.
  • Please fill in as much information as you can (all fields would be preferable).
  • After enrolling, you will receive an email confirmation, but you still have to present yourself at the next circle meeting.
  • We do not require you to enter your parent's or guardian's information, but we strongly recommend that you include it.
